Request to Stay of Infringement

This form should be completed where part of an Annual or Supplementary Fire Safety Statement cannot be submitted by the due date. Example: when testing of one or more fire safety measures is incomplete. 

Request to Stay of Infringement must include a fire engineering report or fire contractor report on what fire safety measure/s is defective, work order from Owners Corporation / Owners to rectify and schedule on when defective fire safety measure/s will be rectified.
